Private Duty Nursing Job Description
Private Duty Nurses are Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) who meet state and federal licensing requirements. They assess and evaluate clients and provide services as indicated by the Plan of Care. Working collaboratively with the Director of Nursing (DON), they update and implement each client’s Plan of Care as appropriate. Private Duty Nurses keep clients healthy and improve the quality of their lives. They engage their clients’ family members to support the overall well-being of their clients. They advocate for their client’s needs with physicians and other ancillary providers. In some cases, they may work with a multidisciplinary team of caregivers to assure their clients receive the level of care and services they need to continue living safely and comfortably in their homes.

Qualifications:
- Graduate of an approved school of professional nursing
- Current licensed Registered Nurse in Massachusetts
- Preferred: BSN or related degree
- One or more years of experience in clinical home health services, or equivalent
- Excellent problem-solving ability, organizational skills and capacity to work independently or as a team
- Demonstrated competence in applying processes designed to ensure adherence to all pertinent federal and state rules and regulations
- Strong communication skills
- Sensitivity to the special needs of older adults and related stress they and their family members may experience
- Demonstrated computer literacy

Collaborative Care Plans & Support
At Connected Home Care, every client is assigned to a Director of Nursing, who oversees his or her care. All clients have Plans of Care that detail the services they need, along with their personal preferences and interests. Private Duty Nurses are matched with older adults that will benefit from the varied skills and experience they offer.
When a Private Duty Nurse is matched with a client, the Director of Nursing helps to assure a smooth transition of care. Connected Home Care is committed to supporting strong relationships between Nurses and their clients and their clients’ loved ones. Depending on the needs and capacities of each client, Private Duty Nurses may recommend adding other caregivers to the team. Other caregivers can include Home Health Aides, Homemakers and Companions. At the direction of the Private Duty Nurse, the team will work together to assess their clients’ ongoing health and well-being. He or she will update Plans of Care to reflect changes in services and oversee the implementation of personal care as appropriate.
Plans of Care have information about a client’s medical needs and his or her ability to perform Activities of Daily Living (ADLs) and other household tasks. For example, a care plan will list:

· Dietary Needs And Meal Preferences
