They engage with clients’ family members to support the overall well-being of their clients. They advocate for their client’s needs with physicians and other ancillary providers. In some cases, they may work with a multidisciplinary team of caregivers to assure their clients receive the level of care and services they need to continue living safely and comfortably in their homes.
