Supportive Care For Alzheimer’s and Related Dementia Care

Led by a Nurse Case Manager, our team follows an individualized care plan to ensure your loved one receives the appropriate level of care, is treated with dignity and respect, and is able to continue living in the comfort and familiarity of his or her home.
Individualized Care Plans
Care Plans have information about a client’s ability to perform tasks and their personal interests.
For example, a Care Plan will list:
- Supports for some or all Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
- Medication schedule
- Dietary needs and meal preferences
- Housekeeping tasks
- Emergency information
- Key contacts
- Names and phone numbers for doctors
- Hobbies, passions, favorite pastimes
People with Alzheimer’s and dementia need varying degrees of support with personal care and household activities. In some cases, clients also need support with mobility and transfers. With their special training, Supportive Aides help clients retain a sense of independence and prevent or reduce confusion and frustration when completing routine tasks. We help people with Alzheimer’s or dementia feel safe and secure. Supportive Aides also provide companionship, help with housekeeping, drive to appointments and run errands.
