Supportive Care For Alzheimer’s and Related Dementia Care

Is a loved one living with Alzheimer’s Disease, dementia or experiencing short-term memory loss? Connected Home Care of Gloucester has a team of specially trained Supportive Care professionals to provide support and help in your loved ones home. Our Supportive Care professionals have special training and experience meeting the often complex needs of people with Alzheimer's Disease and Related Dementia (ADRD), memory deficits and the confusion that can accompany this condition. Led by a Nurse Case Manager, our team follows an individualized care plan to ensure your loved one receives the appropriate level of care, is treated with dignity and respect, and is able to continue living in the comfort and familiarity of his or her home.

Individualized Care Plans

Care Plans have information about a client’s ability to perform tasks and their personal interests.

For example, a Care Plan will list:

  • Supports for some or all Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
  • Medication schedule
  • Dietary needs and meal preferences
  • Housekeeping tasks
  • Emergency information
  • Key contacts
  • Names and phone numbers for doctors
  • Hobbies, passions, favorite pastimes

People with Alzheimer’s and dementia need varying degrees of support with personal care and household activities. In some cases, clients also need support with mobility and transfers. With their special training, Supportive Aides help clients retain a sense of independence and prevent or reduce confusion and frustration when completing routine tasks. We help people with Alzheimer’s or dementia feel safe and secure. Supportive Aides also provide companionship, help with housekeeping, drive to appointments and run errands.
