The Edith Nourse Rogers Memorial Veterans Hospital is a state-of-the-art medical center that provides a broad range of inpatient and outpatient services. The Domestic Violence Services Newtork, Inc. (DVSN) is a unique community collaboration among central Middlesex police departments and non-profit agencies. They train volunteers to proactively advocate for victims of domestic violence and to offer immediate and follow-up crisis intervention services, including risk assessment, safety planning, and connection with supportive community resources

Harvey Wheeler hosts many social and community service activities. The center was recently renovated and shares space with the Concord Children's Center. Health services like blood pressure clinics, social events like movies and meals, enrichment events like interest groups and lectures all take place here.

Minuteman Senior Services is an Aging Services Access Point (ASAP) serving communities in the MetroWest Boston area. ASAPs operate under a state contract to coordinate and deliver a wide range of services to senior citizens in their regions.

Commonwealth Care Alliance serves Massachusetts "dual eligible" population of citizens eligible for both Medicare and Medicaid. CCA provides care for elders and those with disabilities. CCA builds a statewide network of primary care practices and a safety net of human service organizations, independent living centers, and community-based organizations to serve the dual-eligible population.

Pickups start at 1:00 PM. $2.00 charge Thursday: 3rd week of the month - Market Basket, Westford. Pickups start at 12:45 PM $3.00 charge 4th week of the month - Trader Joe's Plaza, Acton. Pickups start at 1:00 PM $2.00 charge Friday: Crosby's Marketplace, Rocky's Ace Hardware and CVS in Concord. Pickups start at 12:40 PM. $2.00 charge WHEELCHAIR VAN The COA also has a wheelchair accessible van. If an individual cannot propel their own chair to and from the van, they must have an escort to assist them. This is a curb to curb service. The Council on Aging can provide transportation on a first come, first served basis to Concord seniors around town for doctor's appointments, trips to the bank, haircuts, visits to a friend, events at the Council on Aging and so on. The Council on Aging van runs Monday through Friday, except for holidays. The first pickup of the day is 8:40 AM and the final pickup to return home is at 3:30 PM. The donation for a van ride, within the town limits, is $2 round trip. Emergency information must be on file in the office for seniors riding on the van. It is important that you call for a reservation in advance. You may sign up at any time. To save room, each shopper is limited to five (5) shopping bags. The van driver will help carry your bags if you are unable to do so.

Weekly Wednesday lunch at NOON - a group from the community caters a nutritious meal. Their is a $2.00 charge. Call the COA, at (978) 318-3020 to make your reservation no later than Monday. On the first Wednesday of each month they celebrate all of the birthdays that fall in that month. After the meal, all are welcome to stay for Bingo.

For seniors who live alone and cannot prepare food for themselves. Volunteers deliver meals and often can provide some social stimulation. Recipients must be at least 60 years old. Services can be for a short period, such as recovery from surgery, or for an indefinite period. Meals are federally subsidized under the Older Americans Act.

THE RIDE paratransit service provides door-to door, shared-ride transportation to eligible people who cannot use fixed-route transit (bus, subway, trolley) all or some of the time because of a physical, cognitive or mental disability. THE RIDE is operated by the Massachusetts Bay Transportation Authority (MBTA) in compliance with the federal Americans with Disabilities Act (ADA). Accessible vehicles are used to serve persons with disabilities, including those who use wheelchairs and scooters. Requests for preferred type vehicles will not be honored. THE RIDE operates 365 days a year generally from 5 AM - 1 AM in fifty-eight (58) cities and towns. For RIDE trips with origin and destination within 3/4 mile of fixed-route service that operates outside of these hours, or in other municipalities within 3/4 miles of MBTA bus service, extended RIDE service is available. The West Area also includes small portions of Billerica and Wellesley that are within 3/4 miles of MBTA bus service. VTS also serves trips to/from the border communities of Chelsea, Everett, Malden, Melrose, Needham, Reading, Stoneham, and Wilmington, as long as one end of the trip is located within one of the non-core area communities listed above.

West Concord Pharmacy is a full service independent pharmacy, and their weekday home delivery service reaches Concord, Lincoln, and Maynard. Free, handicap accessible parking is available. The product line includes prescription items as well as specialty items such as medical supplies, surgical stockings, and homeopathic items.

Carleton Willard Village is a charitable, not-for-profit community offering a flexible range of services, including independent living apartments and cluster homes, assistance in living, skilled nursing and rehabilitation, and support for those with Alzheimer's disease and other dementias.

Concord Park offers independent and assisted living, and is conveniently located adjacent to the train station in the heart of West Concord's shopping village. It is sponsored by the Volunteers of America and is managed by Senior Living Residences of Boston.

Residents have their choice of private studio and one-bedroom apartments and enjoy a variety of beautifully furnished community areas, including a spacious landscaped courtyard and patio. Nashoba Park Assisted Living Community is a Volunteers of America Massachusetts Senior Community, professionally managed by Senior Living Residences of Braintree, MA, a trusted name in senior housing and care with families and eldercare professionals since 1990.

Newbury Court, a not-for-profit residence, has been in Concord since 1994. The community offers a variety of living options and choice in services. Newbury Court provides a continuum of services and amenities as client needs change. Should a client find the need for further assistance with household chores or personal care, rather than trying to find the help, or worse, consider relocation, the community coordinates the provision of those services in the home. If rehabilitation or nursing care should become necessary, Newbury Court is home to Rivercrest Wellness and Rehab. A memory support assisted living neighborhood, The Gardens, is also on site providing a supportive, homelike environment for those with memory loss. Emergency services are also readily available at Emerson Hospital just around the corner.

Youville Place is an assisted living offering private studio and one bedroom apartments for long-term living or short-term respite stays. It is the only mission oriented, not-for-profit assisted living residence in the area. The community offers 92 private studio, one-bedroom and two-bedroom apartments complete with kitchenette, bathroom, and individually controlled heating and air conditioning, and emergency response systems.

BIDHC-Lexington offers patients looking to find a doctor northwest of Boston a convenient location with a broad array of on-site services.
