Connected Home Care provides supportive in-home elder care services to assist senior citizens with care to remain in the home of their choice. We provide 6 levels of elder care, including elder companion care, home-making (meals, light housekeeping, local errands), personal care (help with bathing/showering, dressing, etc.), home health aides, supportive aides specially trained in our program for Alzheimer’s Disease and Related Dementias (ADRD), and skilled nursing services.

SNAP offers nutrition assistance to millions of eligible, low-income individuals and families and provides economic benefits to communities. SNAP is the largest program in the domestic hunger safety net. The Food and Nutrition Service works with State agencies, nutrition educators, and neighborhood and faith-based organizations to ensure that those eligible for nutrition assistance can make informed decisions about applying for the program and can access benefits. For seniors who live alone and cannot prepare food for themselves. Volunteers deliver meals and often can provide some social stimulation. Recipients must be at least 60 years old. Services can be for a short period, such as recovery from surgery, or for an indefinite period. Meals are federally subsidized under the Older Americans Act.
