Personalized care plans:

After her assessment visit, the registered nurse will develop a personalized Care Plan. These Care Plans assure that clients receive the appropriate level of services given their particular health status. Care plans are adjusted regularly to address any changes in status that indicate a need to increase or decrease levels of care. 

Technology:

Technology plays an important role in delivering home health care at Connected Home Care.  Our caregivers are able to check in and out of servicing a client through GPS.  Access and modify care plans and send visit notes and tasks completed to case managers, family members and .  The valuable information can easily be accessed online or sent via family members desired notification setting based on every visit or just when there is something that comes up that they should be aware of.

Caregiver selection:

Each Home Health Aide passes a strict protocol for credentialing and background checks. Throughout their employment with Connected Home Care, HHAs receive ongoing training and skills development in best practices for treating the evolving needs of older adults facing the challenges of aging and associated medical conditions.

Based on a Care Plan a Connected Home Care certified nurse will match your loved one with an appropriate level of caregiver. Our primary goal is to foster lasting, supportive relationships between our caregivers and their clients. We know how important consistent, compassionate care is for the well-being of people as they age and lose the capacity to perform routine activities.

Nurse Case Managers and Home Health Aides continue to work together to assess their clients’ ongoing needs and update personal Care Plans when necessary.
